To confirm your booking, please download and complete the application form (pdf file). Your training or line manager will also need to complete and sign the relevant sections, and sign the back of your passport-sized photograph.
Please read the cancellation policy carefully.
If you will be self-funding, please note this on the form where it requests invoicing details.

Please note that if you wish to claim the discounted fee for members of the IBMS, you must attach a copy of your IBMS registration card to the form.
Fees payable are:
- £650 Members of IBMS and staff from the NHS Trusts and organisations which are part of the Biomed Consortium.
- £750 Full fee
- £900 Fee for overseas students

It is not guaranteed that all modules will be available in every session. If a module should be unavailable, we will contact you to discuss alternatives.
Fees cannot be returned in the event of cancellation by the student after the start date of each course.
